Insurance Information

WHS accepts many Insurance carriers. A routine physical exam includes weight and blood pressure check, hemoglobin testing, urine dip evaluation, and a Pap smear. We gladly submit your insurance claims free of charge as a courtesy to you. To aid us in this process, we need up-to-date information, such as correct name, address, social security number/ID number, date of birth, and most recent insurance card. We cannot verify your insurance coverage/benefits for every service we provide. Some services may not be a covered benefit under your particular plan. He fact that a plan that the plan does not pay for your particular service does not mean that you should not receive it. If your insurance plan determines a service to be not covered, you will be responsible for the complete charge. We will file your claims in a timely fashion with the correct information as provided by the physician. All co-payments are due at the time of service. All other payments are due 30 days of the provided service.
